Job Description

Company Overview:
Solaris is clinician owned and operated and has been recognized as industry experts in palliative medicine, since 1998. We are committed to helping patients and their families always put life first.
Responsibilities and Duties:
Review the list of monthly face-to-face encounters needed. Provide the Face-to-Face Scheduler with the list of encounters and dates needed scheduled within 5 days of receiving the list. Provides a face-to-face encounter no earlier than 30 days prior to the 180-day recertification period and each subsequent recertification as assigned. Attest that such visit took place in accordance with procedures established by the Secretary of the HHS. Completes a narrative based on his/her review of the clinical record or, if applicable, his or her examination of the patient to determine continued eligibility sharing the assessment with the certifying doctor. Documentation must be completed with the dates provided by the Face-to-Face Scheduler. Communicates with Compliance members when a discharge is recommended. Communicates with the Face-to-Face Scheduler for any scheduling or rescheduling needs. This includes but not limited to changes of visit dates, unable to make visit, and unable to document visit within time frame given by Face-to-Face Scheduler. Assists in implementing and monitoring compliance with agency policies and procedures. Participates as a member of IDT meetings, on an as needed basis. Is available as an advisor to hospice staff members, regarding patient care. Consults with physicians about potential hospice patients as indicated.
