  • Per Completed Visit Per-Visit
  • $42 Downtime (pre-approved wait time)
  • $30/hr "Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards. Service AreaSchedule Cleveland / Northeast Ohio | Weekdays, weekends, evenings, on-demand

What You'll Do Provide direct patient care to palliative care patients in their homes in accordance with the established plan of care Monitor and document patient symptoms, comfort levels, and changes in condition Administer medications and treatments as directed by the RN Case Manager or physician Communicate effectively with patients, families, and the interdisciplinary care team regarding patient status and care goals Provide education and emotional support to patients and families managing serious illness Maintain accurate and timely documentation in the EMR within required timeframes What Sets VNA Apart AI-assisted documentation

Requirements Must-Have:
Active LPN license (Ohio) + current BLS certification Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area Minimum 1 year of clinical nursing experience Preferred /
Nice-to-Have:
Prior palliative care, hospice, or home health experience Experience supporting patients and families through serious illness Familiarity with HCHB or similar home health EMR Strong communication and family support skills
