This PRN position offers flexible, as-needed hours, including daytime, evenings, and weekends. The RN will also train, advise, and support caregivers to ensure safe, consistent, and high-quality patient care.

Responsibilities:

Conduct initial assessments of new patients and ongoing reassessments to monitor progress. Provide skilled nursing care for patients with chronic illnesses, including diabetes, heart failure, COPD, hypertension, dementia, chronic kidney disease (CKD), and cancer. Administer medications, implement care plans, and educate patients and families on chronic disease management. Train, advise, and support homecare aides and caregivers, ensuring proper care techniques and adherence to care plans. Collaborate with interdisciplinary care teams to ensure coordinated, patient-centered care. Maintain accurate documentation of nursing care, assessments, and caregiver guidance.

Requirements:

Current RN license in Maryland. Minimum 1 year of homecare nursing experience. BLS certification, Photo ID, and ability to pass a background check. Strong clinical assessment, patient education, and care coordination skills. Excellent communication, leadership, and teamwork abilities. Pediatric care experience is a bonus, but not required.
