We're seeking a compassionate professional with a background in social work, counseling, or public health. The role involves providing direct support, counseling, and case management for individuals affected by STDs and HIV, working closely with healthcare providers and community partners.

Key Responsibilities:

Provide case management and counseling for individuals diagnosed with STDs/HIV Connect patients to testing, treatment, and supportive resources Conduct field investigations and outreach to notify exposed individuals Partner with healthcare providers to ensure quality care and follow-up Offer education and prevention counseling to reduce reinfection Requirements /
