Public Health Nurse/RN

A Full-Time Public Health Nurse/Family Case Manager with a primary focus on the Better Birth Outcomes program is now open for applications of interest. This posting will remain open until filled. The position involves but is not limited to case management of pregnant women, infants, high risk infants and children, including home visiting and may also include EPSDT physical exams of infants and children, immunizations, developmental screenings, perinatal depression screenings and other clinical services. Qualifications:

Licensure in the state of Illinois as a Registered Professional Nurse required; BSN preferred. Experience in and knowledge of public health nursing or population focused nursing preferred
