A QP performs the following duties: This is in Office Position with out in the field for intakes and supervision visits Develop and oversee client's care plan and month-by-month plan Training, orientation, supervision and evaluation of PCA workers Conduct in-person assessments and re-assessments Collaborate with other healthcare professionals such as physicians and social workers to ensure seamless patient care. Maintain accurate and up-to-date patient records and communicating with the homecare agency regarding any changes in the patient's condition Qualifications and Requirements To be considered for a position as a Qualified Professional, you must possess the following qualifications and requirements: Must have RN Licensure in the state of Minnesota. Work experience providing direct care services to persons with disabilities or persons aged 65 and older preferred. Strong communication and interpersonal skills, as well as the ability to work well in a team. Flexibility to travel in the Twin Cities Metro Area Valid driver's license and reliable transportation. Compliance with all relevant state and federal regulations governing home healthcare providers. Bilingual Spanish / English preferred (not required)

Benefits:
