None Typical Working Conditions:
Normal office environment with occasional extended hours. Requires working under stressful conditions and the ability to communicate well with others. Occasionally lifts and carries items weighing up to 50 pounds. Requires corrected vision and hearing to normal range. This position may require patient contact; as well as to support the clinical care team in direct care of our patients.
Qualifications:
Must have healthcare training (LPN, MA, Healthcare education). One year of experience in a clinic setting preferred. Must possess strong understanding of HIPAA rules and regulations, working knowledge of Microsoft Office, and Electronic Health Record (EHR). Ability to maintain professional relationships along with a positive attitude and strong organizational skills.

Primary Function:
The Referral Specialist is responsible for coordinating and completing referrals ordered by providers, acquiring and verifying third party information for referrals, obtaining and arranging appointments, and tracking referrals. This position works with clinical care teams, as well as the Chief Quality Officer, and Health Information Manager, to improve coordination of care for our patients. It requires strong verbal and written communication skills; individuals must be self-motivated and can act independently, as well as part of the team. General Summary of Duties (Referral Specialist): Coordinate all aspects of completing referrals ordered by providers. (i.e., obtain pre-procedure authorizations and insurance authorizations) Contact patients once authorization is obtained. Arrange referral appointments ensuring proper paperwork is given to patient for appointment per orders (i.e., labs, x-rays, comp notes) Close the loop on referrals to ensure follow through (i.e., obtaining records, closing the referral in the EHR) Maintain communication with the provider and the patient regarding the most appropriate specialist to refer to. Maintain specialty provider list including payor sources they accept. Input referrals into computer and generate referral form. Make appropriate notations in the medical records and bring to the attention of the ordering provider and/or clinical coordinator and problem regarding obtaining referrals. Provide assistance to nurses and providers as directed. Perform assignments in accordance with Clinical Policy and Procedure manual. General Summary of Duties (Care Coordinator):
  • Management of patient tracking for diagnostic studies as well as referrals to consulting specialists.
  • Assists the direct care team in the provision of services to patients when this requires coordination with outside organizations.
  • This position requires compliance with Health Center's written standards, including its Standards of Conduct and policies and procedures pertaining to all operational aspects, furthermore this position is required to report any suspected instances of noncompliance.
  • Perform other tasks as assigned.
  • Maintain strictest confidentiality.
Work Location:
As an employee of Family Health Network of Central New York, Inc. you may be assigned to any of our locations - Cortland Family Practice, Homer Family Practice, Marathon Health Center, Cincinnatus Health Center, Moravia Health and Dental Center, Administration, School Health Programs at Marathon, Cincinnatus, or DeRuyter Schools, or our mobile medical unit.
Pay:
$18.50 - $23.00 per hour
