at Home, Hope & Healing in Smithfield, Maine, United States Job Description Home, Hope and Healing is seeking a full-time Regional Nurse Manager (RNM) to be responsible for our home health clients and nursing staff in the Somerset County/midcoast region (Knox, Lincoln, Waldo, some Hancock County). We specialize in 1:1 nursing care for individuals with medically fragile health conditions. This unique and pivotal nurse manager position utilizes a case management model of nursing leadership by case managing a client caseload as well as the team providing care for those clients. This position is responsible for day-to-day clinical operations and management for the designated geographic region. Specific job functions include but aren't limited to: Manage a caseload of clients in the assigned region. Advocate for client and family needs Complete and manage documentation within specified time frames such as authorization requests and plans of care Provide clinical and educational support to staff and clients Participate in process improvement plan to assure that regulatory and agency policies are followed, while modeling organizational values of Compassion, Excellence, Relationships, Integrity and healing Assist and support staff where and when needed Interview, hire and educate nursing staff Performance management for assigned home health team. Make decisions that add value to HHH while achieving quality and fiscal responsibility Establish and implement care plans for new clients
Location:
