Duties Assess patients' nutritional needs through detailed evaluations, including diagnostic assessments and medical history reviews Develop and implement individualized care plans focusing on weight management, long-term health, geriatrics, and acute care needs Provide expert guidance on specialized nutrition therapies such as tube feeding and dietary modifications for various conditions Educate patients on nutrition principles, healthy eating habits, and lifestyle changes to support weight loss and chronic disease management Collaborate with healthcare teams to integrate nutrition strategies into overall treatment plans and monitor progress using electronic medical record (EMR) systems Supervise and mentor junior staff or interns within the dietary department, ensuring adherence to best practices and quality standards Conduct ongoing patient assessments to evaluate progress, adjust care plans as needed, and document outcomes accurately Experience Proven experience in hospital or clinical settings with a focus on geriatrics, acute care, or long-term care environments Demonstrated expertise in nutrition therapy including tube feeding management and specialized diet planning Supervising experience in a healthcare or dietary department is highly preferred Strong knowledge of physiology related to digestion, metabolism, and disease processes affecting nutritional status Hands-on experience with EMR systems for documenting patient information and care plans Background in health coaching to motivate patients towards healthier lifestyles and sustained weight loss goals Familiarity with diagnostic evaluation techniques relevant to nutritional assessment and intervention Join us in making a meaningful difference by empowering patients through expert nutritional guidance.
