Job Description
This posting is for a full time/part time contingent position in a SNF. Responsible for screening, assess, evaluating, educating and monitoring residents meal plans meets the individual's nutritional plan and is effective to ensure the resident highest practicable well-being. Supervisory Responsibilities Provides onsite leadership to facility staff regarding nutrition and facility operations and procedures. Be an active member of the interdisciplinary team and the quality assurance committee. Qualifications Freedom from illegal use of drugs. Freedom from use of and effects of use of drugs and alcohol in the workplace. Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in the position. Working knowledge of personal computer and software applications use in job functions, (word processing, graphics, databases, spreadsheets, etc.) Able to supervise, instructs and analytical skills; oral and written communication skills. Demonstrated ability in independent functioning and strong leadership. Four year college degree or similar higher education required. Current Registered Dietitian License. Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and policy procedure manuals. Ability to effectively present information and respond to questions from groups of executives, managers, clients, customers, and the public. Ability to prepare accurate and timely reporting of travel expenses, automobile mileage records, monthly reports, and calendars. Mathematical Skills Ability to work with mathematical concepts such as statistical process control tools and budgets. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations. Ability to read and interpret financial reports and statistical charts and graphs. Reasoning Ability Ability to define and solve problems collects data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in statistical or diagram form and deal with several abstract and concrete variables. Care and Services:
- Provides service that is kind, patient, compassionate and respectful in accordance with Aerius philosophy.
- Adheres to the facility's policies and procedures as well as state and federal regulations which pertain to the facility.
- Establishes and maintains courteous relationships with referral sources.
- Assures comprehensive initial change of condition and annual nutrition assessment process. MDS, CAA's, care plans, develops, executes and monitors accurately.
- Assure comprehensive initial, change of condition and annual nutrition assessment process/Resident Assessment Instrument (RAI) is initiated, planned developed, executed and monitored accurately and within the time frames specified by state/federal/accreditation requirements.
- Collect nutrition risk data on residents to determine priorities for care. Keep all documentation current.
- Interpret data and information based on age-specific criteria and analyze, address, document and establish treatments and monitoring as appropriate.
- Develop, implement and maintain appropriate departmental and inter-facility policies and procedures.
- Plan, organize, schedule, document and direct individual and group resident and family education experiences. Use age-appropriate materials and methods for determining comprehension and potential for compliance.
- Conduct regular, frequent, ongoing meetings, in-services and training sessions with appropriate departmental and inter-facility staff.
- Plan, write, and implement the resident menu, including modified diet extensions, recipe conversions and production requirements.
- Evaluate, consolidate and manage the department nourishment/supplement and/or central formulary program.
- Monitor telephone log, intake records and other internal documentation/tracking processes.
- Pursue ongoing knowledge and information on disease status to ensure that new procedures are implemented when indicated.
- Translate complex clinical nutrition requirements of residents as part of the healthcare team. Ensure that physicians, nurses and other allied staff understand the nutrition requirements of the resident.
- Develop and nurture a professional working relationship with client, peers and other facility staff.
- Organize, maintain and enforce all local, state, federal accreditation, certification and/or licensure rules and regulation at all times with regard to clinical nutrition care.
- Dedicate time to participate on facility committees and provide follow-ups, as directed.
- Collect data and respond to performance improvement opportunities for continuous quality improvement.
- Speak, listen, and write in a clear, positive and professional manner using interpersonal skills, good judgment and empathy.
- Delegate as appropriate to ensure that job duties are complete.
- Plan, initiate and complete projects to support enhancements to the food service and clinical nutrition program.
- Plan, direct and manage the flow of information and communication from other departments, residents/family and staff.
- Enforce resident rights and confidentiality.
- Write and implement discharge plans for residents.
- Manage the food service department in the absence of the Dietary Manager as directed.
- Mentor, train, coach, and evaluate students, manager associates, interns and other assigned to the department.

Analyzes regional Quality Indicator data, identifies clinical trends, and makes recommendations for improved outcomes in conjunction with the Administrator.Information Management:
Enters or records data timely and accurately, protects confidentiality of sensitive information; protects data against loss or destruction; logs onto the system using own password; logs off the system when leaving the work station; reports suspected violation of security/confidentiality issues; reviews data and identifies trends.Interpersonal Skills:
