Job Description

Santa Anita Convalescent Hospital, Temple City, CA We are seeking a dedicated and knowledgeable dietician to join our healthcare team to provide nutrition service to the largest SNF in southern California. The person will be working alongside our Registered Dietitians, Director of Food and Nutrition Services, Dietary staff, and other disciplinary to provide quality care to our residents and communities.

  • 50/hr, based on experience and degree(s)
Job Description/Responsibilities:
Conduct comprehensive nutrition assessment and reassessment to ensure nutrition adequacy of the residents, including enteral and parenteral nutrition initiation & evaluation, dialysis resident assessment, monitoring and education. Develop, implement and monitor individualized nutritional care plan, including focused issue, goal, intervention, and dietary recommendation. Educate residents, families on medical nutrition therapy and encourage compliance. Participate in IDT weight and wound meetings to address related issue in team approach. Act as liaison among clinical staff, residents, families, and Dietary to encourage optimal nutrition status. Participate monthly in Quality Assurance meeting and perform kitchen sanitation audit, test tray, tray accuracy or other studies to ensure quality of the meals served while sensitive to cultural diversity of the residents. Communicate findings to pertinent parties for quality improvement. Provide necessary in-service to Dietary and Clinical staff. Attend Department Head meeting, clinical meeting, and other duties as assigned Report to the
Administrator Education/Qualifications/Experiences:
Bachelor's Degree in Dietics or nutrition-related field and current CDR license Knowledge of current Federal, State, and local standard/guidelines, policies & procedures, and regulations governing the facility's clinical nutrition and dietary practices. Previous experience in acute care of long-term care setting; at least 2 years highly desirable Excellent communication skills, able to collaborate with team members, and able to adapt to a fast-paced environment while paying attending to detail.
