Job Description
Job Summary We are seeking a dedicated and compassionate Registered Nurse to join our dynamic home care agency. In this vital role, you will provide expert patient care across a variety of medical settings, ensuring the highest standards of safety, comfort, and clinical excellence. Your expertise will support patients through their recovery, manage complex medical conditions, and collaborate with multidisciplinary teams to deliver comprehensive care. Duties Conduct thorough patient initial and subsequent assessments including vital signs, physical examinations, and detailed medical histories using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Alora Plus. Administer medications via various routes including injections, parenteral, and catheter care while adhering strictly to aseptic techniques and HIPAA regulations. Perform specialized procedures such as tube feeding management, vital signs, wound care, etcetera Support patient care by delivering memory care, dementia care, Alzheimer's care, and behavioral health interventions. Provide education and training to other nursing staff, including nursing assistants and home health aides Collaborate with multidisciplinary teams on discharge planning, case management for complex cases such as Workers' Compensation law cases or hospice & palliative medicine patients. Educate patients and families on health coaching topics like medications, nutrition, post-acute care strategies, and chronic disease management including infectious disease care. Maintain accurate medical documentation including ICD coding (ICD-9/ICD-10), CPT coding for procedures, and detailed nursing notes following healthcare regulations. Qualifications Valid RN license in good standing with relevant state licensing boards. Proven experience in diverse clinical settings such as hospital medicine, nursing home medicine, emergency room experience or medical-surgical units. New grads are welcome! Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; proficiency in ICD coding is preferred. Demonstrated ability to perform advanced procedures such as IV insertion/venipuncture, catheterization (urinary or vascular), ostomy care or gastrostomy tube management. Knowledge of physiology principles related to patient assessment and diagnostic evaluation specifically for geriatrics. Experience working with individuals with disabilities or developmental delays; familiarity with memory care or dementia-related conditions is a plus. Strong understanding of infection control protocols and aseptic techniques; experience in acute care environments Excellent communication skills combined with the ability to work effectively within multidisciplinary teams.
