A Home Care Nurse, under nursing supervision and the guidance of the Patient Services Manager/designee, implements the plan of care/ service plan of patients/participants of the Licensed Home Care Services Agency (Agency) in a variety of home settings, and teach health promotion to patients/participants and/or caregivers. The Home Care Nurse provides supervision and direction to paraprofessional staff in the field providing care and services to patients, and participants of Home and Community Based Services (HCBS) Waiver Program(s) of New York State. Also responsible to perform direct nursing service as defined in the job description of the Registered Professional Nurse. The Home Care Nurse is supervised by the Patient Services Manager or designee. The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgement/problem solving, decision making, and initiative. Responsibilities include, but are not limited to, the following: Assess patient/participant care needs and supervise delivery of patient/participant care services to ensure quality of service and maintenance of standards of performance. This shall include consultation, evaluation and documentation of all new and ongoing cases to ensure proper treatment plan. Assists with coordination and management of the total patient/participant care/service plan with appropriate resources, conducting periodic reassessments of patient/participant needs, and revising the plan of care accordingly. Supervise and monitor the performance and skills of field employees to evaluate clinical competence and to assure delivery of quality health care and supportive services through: regular review of patient/participant records; periodic case review conferences with employees; and regularly scheduled as well as discretionary on-site evaluation of employee services in the home. Monitor the assignment of field employees to ensure that it is in accordance with licensure, training, orientation and demonstrated skills. Demonstrate to and instruct aides in appropriate procedures during initial visits, change of personnel and as necessary, or if requested. Assist in orientation, provide initial placement, and provide "on-the-job training" to field employees on an on-going basis as needed. Performance evaluations will reflect demonstration of clinical skills and indicates the need for further "OJT" in specified areas. Instruct field employees as to the observations and written reports to be made to supervisor or therapist. Ensure the maintenance of appropriate reporting and recording mechanisms to assure the documentation of patient care activities/participant services. Reviews daily notes done by field staff, as needed. Participate, as needed, in the training of personal care aides, home health aides, and home and community support services (HCSS) field employees. Conduct supervised practical training requirements for newly trained home health aides in conjunction with the Education Supervisor. Reinforce Agency policies to all field employees. Assist the Assistant Executive Director/designee in the investigation, resolution and documentation of patient/participant or contract agency complaints and incidents. Perform administrative supervision for contract agency cases as required by contractual agreement. Conduct the initial evaluation, assessment and admission of private pay and Department of Veterans Affairs (VA) patients, and HCBS Waiver Program(s) participants to the Agency. Develop and implement the care plan and perform all duties as defined in the Registered Professional Nurse job description. For the HCBS Waiver Program(s), assists the Home and Community Support Services (HCSS) Supervisor (Patient Services Manager/designee) to: oversee and supervise the provision of service and provides direction to the HCSS; maintain communication/collaborates with other Waiver Services Providers regarding participant care and services; and if necessary, participates in participant team meetings conducted by the participant's Waiver Service Coordinator. Maintain current, accurate and complete clinical records, chart promptly changes in patient/participant's condition, adverse reactions and problems with informal supports or home environment and reports such to the Patient Services Manager/HCSS Supervisor/or designee. Progress notes are written, signed and dated following each patient/participant visit or phone contact. When professional care has been provided progress notes contain a summary of patient/participant status and response to plan of care and any contacts with family, informal supports and other community resources. Revisions in the nursing or aide care plan are made as needed, supervisory visits conducted with field staff are recorded. Writes discharge summary for closed cases. Assist as needed in performing periodic employee health status examinations as required by the New York State Department of Health, if needed, and as directed. Assist as needed in administering required tuberculin testing and various immunizations and vaccines to employees, if needed and as directed. Provides constructive and appropriate input in the development of Agency objectives, standards of care, policies and procedures affecting clinical services. Prepare patient/participant records for review by the Clinical Record Review Committee. Report no less than weekly, or as directed, to the Patient Services Manager/HCSS Supervisor/or designee. Performs other Agency related duties as required.Working Environment:
The Home Care Nurse is subject to inside and outside environmental conditions as well as conditions found in patient's homes. The worker may also be exposed to pets, odors, and fumes or mist from medications. The hours of work are determined by availability, work demand and patient need. srnpQualifications:
Education and Training- A current license and registration to practice as a registered professional nurse in New York State. Malpractice Insurance is encouraged. A Baccalaureate Degree in Nursing from an approved program is preferred. Experience
- One year experience in an acute care setting. Home Care Nurse experience is preferred. Character
- The individual must possess good moral character, patience, and compassion for the elderly, chronically ill and disabled.
