Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Collaborate with interdisciplinary teams to develop and implement individualized treatment plans aligned with patient needs. Document all patient interactions thoroughly using agency systems. Monitor patients' conditions providing continuity of care as necessary. Educate patients and families on health management strategies, including medication administration, post-discharge care, and chronic disease management. Requirements Valid RN license in the state of employment with current certification. Proven experience in diverse clinical environments such as hospital medicine, ICU, emergency room, or long-term care facilities. Strong knowledge of medical terminology. Ability to perform physical examinations and assessments confidently while maintaining aseptic technique. Excellent communication skills for effective triage, patient education, case management, and interdisciplinary collaboration. Experience caring for elderly or disabled individuals preferred. Able to travel in the Atlanta metro and surrounding areas.

Pay:

$50.00 - $60.00 per hour
