Registered Nurse Case Manager

We are seeking an exceptional RN Case Manager for our Home Health & Hospice programs. Candidates must be reliable and nursing experience is preferred. Candidates must work efficiently, possess critical thinking skills, and experience with EMR is a plus. We offer a flexible schedule, regularly scheduled 4 days per week and limited on call of every 4th weekend and 1 weeknight every other week. We have a recently revised salary and benefits package, and nurses are provided an agency vehicle for visits.
