Responsibilities Conduct thorough patient assessments including vital signs, physiology evaluations, and medical history reviews to determine appropriate care strategies. Develop and implement individualized discharge plans that incorporate post-acute care options, outpatient services, and community resources. Coordinate with hospital staff, primary care providers, specialists, and managed care organizations to facilitate smooth patient transitions and continuity of care. Manage utilization review processes by evaluating diagnostic evaluations, medical records, and treatment plans to ensure compliance with healthcare policies such as HIPAA. Provide case management support for diverse patient populations including pediatrics, geriatrics, behavioral health patients, and those in ICU or trauma settings. Monitor patient progress through ongoing assessment of vital signs and clinical indicators; adjust care plans accordingly to meet evolving needs. Facilitate communication among patients, families, and healthcare teams to ensure understanding of treatment options and discharge instructions. Experience Proven experience in critical care environments such as ICU or Level I/II trauma centers with exposure to emergency medicine and hospital medicine. Strong background in medical-surgical nursing with familiarity in infectious disease care, post-acute settings, outpatient clinics, and inpatient units. Demonstrated expertise in patient assessment techniques including vital signs monitoring and physiological evaluation. Knowledge of medical terminology, diagnostic evaluation procedures, discharge planning processes, and utilization management protocols. Experience working within managed care frameworks or healthcare systems emphasizing patient service excellence. Familiarity with hospital experience requirements across various specialties including PICU (Pediatric Intensive Care Unit), OR (Operating Room), Geriatrics, Behavioral Health, and outpatient clinics. Ability to navigate complex medical records systems while maintaining compliance with HIPAA regulations; proficient in medical records documentation.
