Job Summary:
We are seeking an experienced RN Case Manager to join a high-volume acute care hospital in Eureka, CA. The RN Case Manager will be responsible for care coordination, discharge planning, utilization review, and collaborating with interdisciplinary healthcare teams to ensure patients receive appropriate and timely care throughout their hospitalization. This role requires strong acute care hospital case management experience, excellent communication skills, and the ability to navigate complex patient care coordination and insurance authorization processes.
Duties:
Perform acute care hospital case management and utilization review Coordinate patient discharge planning and transitions of care Collaborate with physicians, nursing staff, social services, and interdisciplinary teams Review admission criteria and medical necessity documentation Handle appeals, denials, and prior authorizations Coordinate DME needs and post-discharge resources Ensure compliance with CMS, HIPAA, Joint Commission, and regulatory guidelines Document patient care coordination activities within Epic EMR Assist with benefits eligibility verification and payer communication Float between departments based on facility needs
Requirements:
Minimum 2 years of acute hospital RN Case Management experience required Current acute care case management experience within the last year required Active RN License required Epic charting experience required Experience with discharge planning and utilization review required Experience with appeals and denials preferred Travel experience preferred First-time travelers accepted with strong background
