What You'll Do Perform comprehensive patient assessments and create individualized care plans Manage a caseload of hospice patients, coordinating care with the interdisciplinary team Administer medications and treatments per physician orders Educate patients and families on symptom management, medication, and end-of-life care Monitor and update care plans based on ongoing patient assessments Provide emotional and clinical support to patients and caregivers Communicate with physicians, care partners, and community resources Maintain accurate, timely clinical documentation Participate in interdisciplinary team (IDT) meetings and on-call rotation Supervise ancillary staff and ensure continuity of care To view full details and how to apply, or
