Registered Nurse Case Manager

Registered Nurse (RN) Case Manager Full-Time/Part-Time / PRN Competitive Pay Based on Experience We are seeking a compassionate and dependable Registered Nurse (RN) with home healthcare experience to join our healthcare team. The RN will play a key role in providing direct patient care, coordinating treatment plans, and supporting the overall health and recovery of our patients.

Location Warren County, Ohio and Butler County, Ohio Key Responsibilities:

Assess (OASIS) and monitor patient conditions Administer medications and treatments as prescribed Collaborate with physical therapists and healthcare team members Document patient progress and maintain accurate records Educate patients and families on care plans and health management Supervision of Clinical (LPN/HHA) Staff in the field.

Requirements:

Active RN license in Ohio CPR/BLS certification required Strong clinical and communication skills Experience in a rehabilitation or outpatient setting is a plus Pay rate for

PRN/Per-diem:
