Scuffletown Rd, Simpsonville, SC 29681 Part-time Part-time Join our compassionate healthcare team as a Hospice Registered Nurse, where your expertise will provide vital end-of-life care that emphasizes comfort, dignity, and personalized support for patients and their families. In this dynamic role, you will deliver comprehensive nursing services in patients' homes, clinics, or inpatient settings, ensuring seamless coordination of care and adherence to best practices. Your critical care background and extensive clinical experience will drive meaningful outcomes, offering solace and professionalism during some of life's most sensitive moments. Responsibilities Conduct thorough patient assessments, including vital signs, symptom management, and physiological evaluations to determine individualized care plans. Deliver expert nursing care across various settings such as inpatient units, outpatient clinics, or post-acute environments, ensuring optimal patient comfort and safety. Collaborate with interdisciplinary teams to develop and implement personalized discharge planning and ongoing case management strategies. Manage medical records accurately while adhering to HIPAA regulations to protect patient confidentiality at all times. Perform diagnostic evaluations and coordinate diagnostic testing to inform treatment adjustments and symptom control. Engage in intake assessments for new hospice patients, ensuring timely initiation of appropriate care services. Provide education on disease processes, medication management, and symptom control techniques tailored to patient needs. Demonstrated expertise in hospital medicine, medical-surgical units, or post-acute care facilities with a focus on geriatrics or behavioral health. Strong knowledge of physiology, infectious disease care, and medical terminology necessary for complex patient assessments. Prior experience in hospice or palliative care is preferred but not required; relevant outpatient or clinic experience is advantageous. Ability to perform comprehensive patient evaluations including vital signs and diagnostic evaluation techniques. Familiarity with HIPAA compliance, medical records management, discharge planning, utilization management, and case management processes. Excellent communication skills with a focus on patient service and compassionate bedside manner.
