Job Description
Develops, coordinates, implements and evaluates the plan of care with patients s/p CABG, Valvular repair or replacement, MI, PTCA with or without STENT placement AND Stable angina. Instruct patients on appropriate low-level activity as per department policy & procedure. Evaluates patients cardiac status before, during and after exercise, including instruction and supervision of a structured progressive monitored activity program for all candidates. Organizes and conducts patient and family groups for the purpose of supportive counseling, exercise limitations and coronary artery disease.

Young Adult- Geriatric. Provides a Safe and Stimulating Environment for Patients and assumes responsibility for the total needs of the patients including physical, psychological and education daily. Assesses patients needs, sets priorities, and evaluates EKG, vital signs, and other responses to exercise daily. Adapts present exercise regime to sudden change in cardiac status. Recognizes emergency situations and reacts appropriately. Demonstrates customer focus care to all patients/families/significant others. Checks and calibrates all equipment, cleans and maintains equipment and notifies clinical engineering for appropriate services.Age Specific Competencies:
Young Adult-Geriatric. Assess and evaluates clients Psychosocial needs and initiates referrals to professional community resources. Addresses each patients problems according to their level of coping, denial, anger and acceptance. Encourages verbalization and problem solving while educating patients regarding professional counseling services and support groups. Ensures that patients rights are respected and complies with HIPAA guidelines outlined in AHS policy & procedures.Age Specific Competencies:
Young Adult Geriatric. Functions as an educator for Patients, Family, community and colleagues. Assess patient and family educational needs. Provides education on risk factors, behavior modification, life style changes, disease process, and use of medication. Conducts formal and informal, individual and group teaching sessions for patients and families. Flexes teaching style to meet a variety of learning needs. Provides orientation for new staff, acts as a mentor to students and internsAge Specific Competencies:
Adult Geriatric. Checks emergency equipment daily for proper function and safety; contacting the Bio-Medical department in the event of malfunction. Checks exercise equipment daily for proper function and safety. Documents malfunction, tags equipment, and notifies Bio-Medical department. Documents any physician notification or nursing intervention in the clients chart and the departments Untoward Event logbook. Documents incidents and accidents fully in accordance with hospital policy. Facilitates education activities such as bulletin boards, lectures, games and National Heart Week programs. Participates in Shared Goverance programs and annual PACT clinical ladder as eligible. Works with Medical Director to facilitate ITP and multidisciplinary plan of care. Assists Cardiologist in documenting in the Physician Supervision Log.Age Specific Competencies:

Assisting patients scheduling initial and follow up appointments. Documents are pertinent patient information in the electronic medical record. Accurately reflects the patient appointments on the electronic schedule as well as maintains functionality of the schedule.Physical Requirements:
