Position Summary :

Responsible for providing professional nursing care to patients following established standards and practices in the home setting. Functions as primary clinical liaison between patients and physicians.

Essential Functions:

Performs initial and ongoing assessment of physical, psychosocial and environmental needs of patients in the home setting Confirms assessment findings with physicians and completes documentation Develops therapeutic nursing interventions to meet patient's individual goals and determines the need for additional skilled services Provides supervision for home health aide and licensed practical nurses as appropriate per the policies of HomeCare Maryland LLC and the Maryland regulatory agency Ongoing interdisciplinary assessment of the patient

Qualifications/Requirements:

Registered Nurse License for the state of Maryland Current American Heart Association BLS for healthcare providers Prior experience in home care preferred, but not required 2-3 years as a Registered Nurse 2-3 years acute care setting experience HomeCare Maryland is an equal opportunity employer.
