HCBS AMH
Nurse provides direct and indirect nursing services to adults with serious mental illness enrolled in the Home and Community-Based Services Adult Mental Health program. This role ensures the delivery of trauma-informed, person-centered care that promotes recovery, independence, and community integration. The nurse collaborates with interdisciplinary team members to maintain compliance with state and federal regulations, safeguard resident health, and support individualized service plans. Key Responsibilities Conduct comprehensive nursing assessments and document findings in compliance with
HCBS AMH
standards. Develop, implement, and monitor individualized care plans in collaboration with residents, families, and interdisciplinary staff. Administer medications safely, maintain accurate medication records, and oversee refill protocols. Provide health education and training to residents and staff on wellness, chronic disease management, and crisis prevention. Monitor residents' physical and mental health status, intervening promptly in emergencies or changes in condition. Ensure compliance with infection control, safety protocols, and state licensing requirements. Participate in interdisciplinary team meetings, contributing nursing expertise to service planning and progress reviews. Maintain accurate, timely, and legally compliant documentation, including progress notes, incident reports, and care updates. Support trauma-informed practices that respect resident autonomy, dignity, and recovery goals. Provide staff training and guidance on health-related procedures, medication administration, and crisis response. Qualifications Current license as a Registered Nurse (RN) in the State of Texas. Experience in mental health, psychiatric nursing, or community-based care preferred. Knowledge of
HCBS AMH
program requirements, Medicaid regulations, and traumainformed care principles. Strong organizational, communication, and documentation skills Ability to respond effectively to crises and collaborate with interdisciplinary teams. Commitment to ethical, professional, and compassionate care delivery. Working Conditions Community-based and residential care settings. Flexible scheduling, including evenings, weekends, and on-call responsibilities as needed. Requires mobility to visit multiple program sites and residents in community settings.
