assessments Manage a caseload of patients across our service area with autonomy and physician collaboration Document accurately and timely in Axxess EMR (training provided) Coordinate care with LVNs, therapists, MSW, and HHAs Educate patients and families on disease management, medications, and safety Participate in case conferences and QAPI initiatives Must-haves Active, unencumbered Texas RN license (or compact license valid in TX) 1+ year of clinical nursing experience (any setting) Current CPR/BLS certification Valid Texas driver's license, auto insurance, and reliable transportation Ability to lift 50 lbs and perform hands-on patient care in the home Nice-to-haves Home health experience and

familiarity Bilingual English/Spanish (strongly preferred — most patients are Spanish-speaking) Experience with Axxess Wound care, IV therapy, or diabetic management certifications
