36 hours/week Position Overview "Navitas Healthcare, LLC" is seeking a Registered Nurse (RN) to provide skilled nursing care in home health, hospice, and palliative care settings. The RN will be responsible for delivering patient-centered care in home environments, coordinating with physicians and interdisciplinary teams, and managing individualized care plans to support patient comfort, safety, and quality of life. Key Responsibilities Perform initial patient assessments and ongoing reassessments Develop, implement, and update individualized care plans Provide skilled nursing care in home health and hospice settings Document clinical notes and patient progress in Epic EMR Coordinate care with physicians and interdisciplinary team members Educate patients and families on care plans, medications, and disease management Supervise and support other nursing staff as needed Participate in quality improvement initiatives and in-service training Provide end-of-life care support with dignity and compassion Required Qualifications Active Registered Nurse (RN) license Minimum 3 years of nursing experience

BLS/BCLS
