Job Description

Overview Join our dynamic healthcare team as a Registered Nurse specializing in Skilled Nursing Facility (SNF) care! In this vital role, you will deliver compassionate, high-quality nursing services to residents in a long-term care environment. Your expertise will ensure optimal patient outcomes, promote comfort, and support the overall well-being of individuals with diverse health needs. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories. Administer medications and injections with precision, following all safety protocols and HIPAA regulations. Manage complex care plans involving IV infusions, wound suturing, catheter care, and tube feeding. Monitor patients' conditions closely, utilizing advanced EMR (Electronic Medical Records) systems such as Epic or Cerner for accurate documentation. Provide specialized care for residents with memory care needs, dementia, or behavioral health challenges. Coordinate discharge planning and collaborate with interdisciplinary teams to ensure seamless transitions of care. Implement infection control procedures and aseptic techniques to maintain a safe environment for residents and staff. Experience Proven experience working in long-term care facilities such as nursing homes or assisted living communities. Critical care experience in settings like ICU or trauma centers is highly valued. Familiarity with medical procedures including cardiology interventions like cardiac catheterization and ostomy care. Hands-on experience with EMR/EHR systems such as Athenahealth or eClinicalWorks is preferred. Knowledge of ICD coding (ICD-9/ICD-10), CPT coding, and medical documentation standards. Demonstrated ability to perform complex patient assessments involving physiology knowledge and diagnostic evaluation. Prior roles may include hospital medicine, emergency room experience, or post-acute care settings that emphasize acute pain management and trauma medicine.
