No Weekends Rehabilitation Manager Job Summary:
Day Kimball Health is seeking a Rehabilitation Manager (Homecare) to join our Homecare team in Thompson, CT. This leadership role is responsible for overseeing rehabilitation services in the home health setting, ensuring high-quality, patient-centered care, and supporting the effective delivery of therapy services across disciplines. The Rehabilitation Manager serves as a clinical leader and supervisory resource for Physical Therapists, Occupational Therapists, Speech-Language Pathologists, Therapy Assistants, and students . This role is responsible for coordinating rehabilitation services, ensuring regulatory compliance, optimizing therapy resource utilization, and driving consistent, high-quality outcomes for patients receiving home-based care.
Rehabilitation Manager Key Responsibilities:
Accountable for maintaining established productivity goals, continuity of care, and appropriate utilization of resources. Supervises patient care practices provided by the therapy team, acting as a clinical resource. Provides direct supervision for all homecare therapy staff. Completes annual performance appraisals and actively mentors reporting employees. Sets performance goals for staff, monitors progress, and provides feedback and the resources needed to develop required skills. Hires, terminates, monitors, and evaluates the performance of individual staff, holding them accountable to established policies, procedures, and regulatory requirements. Functions as a member of the homecare leadership team, participating in policy development, establishing and implementing best practices, and is accountable for compliance with therapy state, federal, and accreditation requirements. Participates in Quality and Performance Improvement initiatives including, analysis, development, auditing, and participation in quality improvement teams and committees within homecare and the larger Day Kimball Health System. Manages Day Kimball Homecare's Fall Prevention Grant Program, including assisting with annual budgeting and fiscal monitoring activities. Demonstrates knowledge and skills necessary to provide direct patient care as needed, ensuring care provided is appropriate to patient age, developmental needs, sex, gender, religion, culture, race, and ethnicity.
Rehabilitation Manager Qualification Requirements:
Specific vocational preparation includes an occupationally significant combination of education, training, and on-the-job training. Minimum 3 years of experience in a clinical practice setting and one year of clinical Physical Medicine and five years related professional experience required. CT license or CT license eligible required. CPR Certification required or within three months of hire.
Minimum:
BS degree in Physical Therapy, or Occupational, or MS or MA degree and a Certificate of Clinical Competence) for Speech Therapy required.
