RN Hospice in Patient All Shifts - Per Diem East End Hospice is currently seeking an experienced Registered Nurse for a Per Diem opportunity providing high quality hospice nursing services to East End Hospice patients and their families within its inpatient unit, The Kanas Center for Hospice Care Responsibilities of a

Hospice RN:

Teach, supervise and counsel the patient and family regarding care and other problems related to the patient's illness. Provide anticipatory guidance to patients/families to assist them in preparing for and coping with the events as they occur throughout the disease process. Conduct comprehensive, individualized patient assessments. Contribute as a member of the East End Hospice interdisciplinary team to identify the patient/family needs and to establish the plan of care. Implement the nursing portion of the plan of care in coordination with the other members of the interdisciplinary team. Share in the on-going evaluation of the plan of care and coordinate revisions as needed.

Qualifications:

Ability to relate well to terminally ill patients and their families. Ability to assess patient/family needs and to be sensitive to those needs. Willingness to function as a member of the interdisciplinary team. Be familiar with the hospice concept and related nursing issues.

Requirements:

A registered professional nurse from an accredited nursing program. Currently licensed to practice nursing in New York State. 2+ years of RN experience required (Hospice, Med-Surg, Homecare) Experience with Electronic Medical Records a plus

Schedule:

7pm to 7am/7:00 am to 7:00 pm Weekends will be required on a rotating basis Valid New York State driver's license and proof of current car insurance required for all positions. East End Hospice offers competitive salary and benefits, supportive working environment.

Pay:

$55.00 - $60.00 per hour

Education:

Associate (Preferred)

Experience:

Nursing (Hospice, Med-Surg, Homecare): 2 years (Preferred)

License/Certification:

New York State RN Professional License (Required)
