JOB SUMMARY
This role is part of our exciting expansion plans in Rhode Island. The individual will be responsible for performing initial client evaluations, developing the Service Plan/Plan of Care, and conducting supervisory visits in the client's place of residence in Rhode Island.RESPONSIBILITIES
Essential job duties for this position include the following items. Other duties must be performed as assigned or required. Conduct client referrals/intakes Perform initial and ongoing inhome evaluation, caregiver introductions, coordination and supervision of client services Initiates appropriate preventive and rehabilitative nonmedical procedures; may provide limited skilled nursing services according to FirstLight's service boundaries, when applicable Train caregiver Complete appropriate visit records in a timely manner and according to policy Conduct initial and ongoing visits to prospective and existing clients following initial referrals to introduce FirstLight Home Care and foster relationship growth. Participate in the quality assurance reviews and evaluations of the agency's services, as requested. Stay current with changing technology, including software programs Uphold, support, and promote all company policies and proceduresQUALIFICATIONS
To perform this job successfully, the individual in this position must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills, abilities, certifications, and educational experience required.EDUCATION/EXPERIENCE
Graduate of a stateapproved school of nursing and currently licensed in the state(s) in which practicing. Minimum of one (1) years' experience in nursing, home care experience preferred. Proficient skills in Microsoft Office and or Google Suite and scheduling software required Must possess and demonstrate excellent organizational, communication, interpersonal and leadership skills as well as a positive and professional imageCERTIFICATIONS, LICENSURE, & REGISTRATION
Maintain current state Registered Nurse (RN) licensure. Must have a valid driver's license and auto insurance.LANGUAGE SKILLS
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from team members, clients, customers, and the general public.REASONING ABILITIES
Ability to evaluate situations and make complex decisions using critical thinking and reasoning.PHYSICAL DEMANDS & WORK ENVIRONMENT
