RN - Medicare Wellness (Hoopeston)

Overview $5,000 Sign on Bonus Available!! $2,500 Relocation Bonus Available (greater than 50 miles) (external candidates only) The Ambulatory Nurse utilizes a systematic approach to nursing by incorporating assessment, planning, implementation and evaluation in the practice of nursing. Provides healthcare to individuals, families, and communities within the RN scope of practice as defined by the Illinois Board of Nursing. Main focus of the Medicare Wellness nurse will be to complete Medicare Annual Wellness visits with eligible patients; in addition, help with clinic nursing responsibilities as defined or needed.

Qualifications Certifications:

Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR); Basic Life Support (BLS) within 30 days - American Heart Association (AHA),

Responsibilities Ensures understanding of CMS Medicare Annual Wellness Visit requirements/visit workflowsProvides and facilitates competent health care of patients and families in area of practice. Ensures adherence to established standards of care and scope of practice.

Ensures documentation of care is complete and accurate.

Provides health and wellness education to individuals and groups.

Acts as preceptor and supports the development of other staff and healthcare students.

Provides leadership for initiatives designed to improve the quality of care or clinical outcomes.

Assists Nursing Supervisor/coordinator in the development of departmental standing orders.

Works with Nursing Supervisor to ensure department is compliant with DNV standards.

Perform telephone visits with eligible Annual Wellness Visit patients Complete AWV as assigned per departmental benchmarks/guidelines Assist with coordination of care for patients age 18-150. Perform telephone triage of both acute chronic needs for patients age 18 -150. Assist with in office procedures, such as incision drainage, joint injections. Compensation and Benefits The compensation range for this position is $30.39per hour - $52.27per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
