The Wound Nurse is responsible for overseeing and providing comprehensive wound care services to residents, with a strong focus on prevention, treatment, healing, and education. This role works closely with physicians, nursing staff, therapy, dietary, and interdisciplinary teams to ensure the highest quality of care for residents with pressure injuries, surgical wounds, diabetic ulcers, skin tears, and other chronic or acute skin conditions. The ideal candidate is passionate about geriatric care, knowledgeable in current wound care practices, and committed to improving resident outcomes while maintaining regulatory compliance and infection control standards. Essential Duties & Responsibilities Perform comprehensive wound and skin assessments upon admission, daily treatments, weekly, and as needed. Develop, implement, and monitor individualized wound treatment plans in collaboration with physicians and interdisciplinary team members. Provide direct wound care treatment including dressing changes daily, measurements, wound staging, and pressure injury management. Monitor and document wound healing progress accurately within PointClickCare in accordance with facility policy and state/federal regulations. Ensure accurate completion of wound documentation, physician orders, treatment records, progress notes, and weekly skin reports. Ensure compliance with infection control procedures, OSHA guidelines, and CMS nursing home regulations related to wound care and skin management. Conduct routine skin rounds and participate in Quality Assurance and Performance Improvement (QAPI) initiatives related to wound prevention and treatment. Educate nursing staff on proper wound care techniques, skin integrity protocols, pressure injury prevention, repositioning, and infection control practices. Collaborate with dietary services regarding nutritional needs that support wound healing and skin integrity. Coordinate care with physicians, nurse practitioners, therapy staff, hospice providers, and outside wound specialists as needed. Monitor residents identified as high risk for skin breakdown and implement preventative interventions promptly. Participate in resident and family education regarding wound prevention, treatment plans, and healing expectations. Assist during state survey inspections and maintain survey readiness related to wound care documentation and regulatory compliance. Maintain inventory and appropriate use of wound care supplies and specialty equipment. Weekend on-call rotation with Nursing Managemnet Team Scheduling Transportation for Upcoming appointments Qualifications
- Current and active RN license in the state of employment.
- Previous wound care experience in a skilled nursing facility, long-term care, rehabilitation, or hospital setting preferred.
- Strong knowledge of pressure injury staging, wound assessment, infection prevention, and skin management protocols.
- Experience caring for geriatric residents, including those with dementia, diabetes, mobility limitations, and chronic medical conditions.
- Experience using PointClickCare for nursing documentation and charting preferred.
- Knowledge of CMS regulations, long-term care standards, and nursing home compliance requirements preferred.
- Excellent assessment, communication, organizational, and time management skills.
