What You'll Do Provide skilled nursing visits in homes and community settings. Perform assessments, wound care, and medication management . Educate patients and families on care plans and recovery. Adapt quickly to new territories and patient needs. Collaborate with RN Case Managers and interdisciplinary teams. Maintain accurate, timely documentation . What We're Looking For Active Minnesota Registered Nurse (RN) license. Strong clinical judgment and willingness to learn. Comfortable working independently in a field-based role .
