Job Description
The Community & Hospital Liaison is a field-based business development professional responsible for building, maintaining, and growing referral relationships with hospitals, physicians, discharge planners, case managers, social workers, assisted living communities, and other healthcare partners within an assigned territory. This role is the external face of the organization — driving census growth through consistent presence, strategic relationship development, and deep knowledge of the organization's clinical capabilities and center offerings. The Community & Hospital Liaison works in close partnership with the Central Clinical Liaison team and Directors of Admissions to ensure a coordinated, high-quality referral experience from first contact through admission. Key Responsibilities Territory Management & Referral Development Develop and execute a strategic territory plan targeting key referral sources — hospitals, SNF discharge teams, physician groups, home health agencies, assisted living communities, and outpatient providers Conduct consistent, scheduled weekly visits to referral sources across the assigned territory Build and maintain authentic, long-term relationships with discharge planners, case managers, social workers, and physicians to establish the organization as the preferred post-acute placement partner Identify and cultivate new referral sources to expand the organization's market presence Referral Source Education & Communication Educate referral sources on the organization's clinical capabilities, specialty programs, payer mix acceptance, and available services at each center Communicate any changes in center capacity, specialty offerings, or admissions criteria to referral partners proactively and promptly Serve as a knowledgeable resource for referral sources navigating the post-acute placement process Pending Referral Support Go bedside at the hospital to visit patients and families on pending referrals — build relationships, answer questions, and represent the center with warmth and professionalism Collaborate with the Director of Admissions and Central Clinical Liaison Pod on pending referral follow-up strategy Support the family engagement process by reinforcing the center's strengths and differentiators during bedside visits Community Presence & Events Attend relevant community events, health fairs, hospital events, and networking functions to expand the organization's visibility and referral network Represent the organization's brand, values, and culture in all community interactions Collaboration & Reporting Brief the Central Clinical Liaison Pod when a referral from a new or high-priority source is anticipated — enable proactive preparation Collaborate with Directors of Admissions at assigned centers to align community messaging with center-level capabilities and current availability Track all account activity, referral source visits, relationship notes, and outcomes in the CRM — maintain accurate, current records Provide weekly territory activity reports to the VP of Business Development including referral volume, relationship status, new account development, and competitive intelligence Meet or exceed defined performance benchmarks including minimum weekly visit counts, referral volume targets, and conversion goals Qualifications & Requirements Education & Experience Minimum 2 years of experience in healthcare sales, marketing, or business development — required Experience in skilled nursing, post-acute care, or healthcare field marketing strongly preferred Existing relationships with hospital discharge planners, case managers, physicians, or social workers in the local market a significant plus Clinical background preferred but not required — working knowledge of SNF clinical capabilities and payer landscape beneficial Skills & Competencies Proven ability to build and maintain professional relationships in a healthcare or business development context Strong presentation and communication skills — comfortable engaging with clinical staff, hospital leadership, and community partners Self-motivated, results-driven, and able to manage a full territory calendar with minimal oversight Competitive awareness — able to identify market opportunities and communicate the organization's differentiators effectively Proficient in CRM platforms for account tracking, activity logging, and pipeline reporting Valid driver's license and reliable transportation — daily territory travel required What Success Looks Like in This Role A successful Community & Hospital Liaison is known by name at the hospitals and physician offices in their territory. They show up consistently, bring value to every interaction, and make referral sources feel confident sending patients to our centers. They are a connector — between the community and the pod, between the family and the center, between opportunity and admission. Their territory produces a growing, reliable referral stream that reflects the strength of the relationships they've built.
