Full Job Description Requirements:
  • Must have completed two full years full-time study at an accredited college or university or an individual with a high school diploma or GED. May substitute one-year full-time employment in a supervisory capacity un a health care facility, agency, or community based agency for each required year of college.
  • Experience in Texas PAS programs desired.
  • Must have excellent time management skills.
  • Appreciation for the needs and characteristics of the client population.
  • Must have skills in problem solving/interpersonal communication.
  • Must have a range of supervisory skills in order to implement referred services and follow through.
  • Develop excellent communication skills and build a team approach with HHSC/Contractor/In-house staff.
  • Must be able to read, write and comprehend English.
  • Must have a valid Texas Driver's License.
Responsibilities:
Include but not limited to
  • Assess client's functional status and take necessary action to ensure client's well-being.
  • Promote, review, and evaluate the quality and appropriateness of client care practices.
  • Ensure services are to be delivered in full compliance with the
HHSC/CONTRACTOR
regulations, contract standards, and agency policy and procedures.
  • Must be able to assume a leadership role, function independently and participate in back-up supervisory casework both in giving and receiving.
  • Recruit, interview, hire, train, supervise, evaluate, and terminate (if needed) attendants needed to provide services to clients.
  • Make initial visits to each client's home with the attendant on/or before initiation of service for the purpose of developing a plan of care, orientation, and hiring the attendant.
  • Complete all paperwork belonging in client charts and copy to clients.
  • Review on-going evaluation of performance of attendants.
  • Receive and act to resolve complaints made by clients/families regarding care provided relating to services, personnel or policies.
  • Inform and confer with PHC Director/Designee on all client, personnel and service management needs and problems.
  • Participate in assigning attendants to clients based on client needs.
  • Assist PHC Director/Designee in audits by State and Federal government.
  • Project a concerned, professional attitude and develop a positive rapport with community resources, caregivers, and all staff.
  • Coordinate and ensure that the client is well services through the assignment of personal caregivers whose skills and abilities meet their individual needs.
  • Assure confidentiality of all client, physician, and propriety information.
Participate in all quality improvement activities, as required.
