Patient Facilitator (75697) PA, Mercer - State Correctional Institution Mercer - Mercer, PA 16137 Apply Overview Position Type Per Diem Job Shift Day Education Level High School Diploma/GED Category Behavioral Health Professional & Support Apply Description $17.50-18/hour based on years of experience Centurion is proud to be the provider of mental health services to the Pennsylvania Department of Corrections. We are currently seeking a PRN Patient Facilitator to join our Team at State Correctional Institution Mercer located in Mercer, PA. The Patient Facilitator will assist with the flow of patients in and out of the TeleHealth clinic and remain present during the clinic session. Other responsibilities will include educating the patient on the nature and process of TeleHealth, navigating and reviewing patient information using the Electronic Medical Records system, and maintaining and assisting the provider in using the TeleHealth equipment.
