Patient Liaison Life Care Home Health - 3.2 San Bernardino, CA Job Details Full-time | Contract | Per diem $300 a day 17 hours ago Benefits Work from home Flexible schedule Qualifications Hospice experience Driver's License Home health agency experience Stakeholder relationship building Full Job Description We are seeking a motivated and compassionate Community Liaison / Outreach Coordinator to support awareness of our home health and hospice services in the local community. This role focuses on building relationships, providing education, and connecting individuals with appropriate care resources. Responsibilities Engage with community members, families, and referral sources in a professional and respectful manner Provide general education about available home health and hospice services Build rapport and ensure individuals feel comfortable asking questions about care options Identify individuals who may benefit from home health or hospice services Collect basic contact information only from individuals who voluntarily express interest Coordinate referrals to the intake/admissions team for further evaluation and follow-up Represent the organization at community events, facilities, and outreach activities as needed Maintain accurate records of outreach activities and contacts Ensure all interactions are conducted with professionalism, dignity, and respect Qualifications Minimum 2+ years of experience in home health or hospice required Experience in healthcare outreach, patient liaison, admissions, or care coordination strongly preferred Strong communication and relationship-building skills Compassionate, professional, and patient-focused demeanor Comfortable engaging with families in sensitive situations Basic understanding of home health or hospice services Valid driver's license and reliable transportation (if community travel is required) Proficiency with basic computer systems and documentation tools Compensation Base pay + performance-based incentives available (structure discussed during interview)
