Canyon Creek Post-Acute is a 5-star Customer Satisfaction Management (CSM) rating, 70-bed skilled nursing facility located in Castro Valley, California, specializing in short-term rehabilitation and long-term care. Our mission is to provide exceptional services that enable our residents and their families to thrive. With a dedicated in-house team of therapists and nurses, we create customized care plans tailored to the individual needs of each resident.
Position Overview:
We are seeking an experienced and dedicated Nurse Supervisor to join our skilled nursing facility team. The main responsibility is overseeing the delivery of high-quality patient care, ensuring compliance with all regulations, and providing leadership to nursing staff. Also should be responsible in creating a positive work culture that promotes mutual respect, teamwork, and continuous improvement. This is an exciting opportunity for a passionate nursing leader looking to make a significant impact in a rewarding environment. Supervise and evaluate nursing staff to ensure compliance with care standards and regulations. Develop and implement individualized care plans for residents in collaboration with the healthcare team. Monitor patient health status and provide clinical guidance to nursing staff as needed. Conduct regular staff meetings and facilitate effective communication among team members. Ensure the facility maintains high standards of patient safety and quality of care. Participate in staff training and development, providing mentorship and guidance to junior nurses. Manage patient admissions, discharges, and transitions of care to ensure seamless services.
Qualifications:
Valid and unrestricted RN license in the state of practice (California) is a must Minimum of 2 years of nursing experience in a skilled nursing or similar facility is preferred Previous Nursing supervisory experience in a healthcare setting is strongly preferred. Strong knowledge of state and federal healthcare regulations. Excellent communication, organizational, and interpersonal skills. Ability to work effectively in a fast-paced and dynamic environment.
