with any questions or call (317)740-7731 Duties Develop and implement outreach strategies to engage community members and promote programs. Educate individuals and groups about available resources, services, and programs related to in home rehabilitation. Collect and analyze data via Trella to assess impact and potential referral sources. Collaborate with local organizations, hospitals, rehabilitation centers, skilled nursing facilities, assisted/independent living facilities, physicians, etc. Oversee project management tasks to ensure timely initiation of hospice services. Provide administrative support as needed, including maintaining records and preparing reports. Experience Proven experience in community outreach and business development within the hospice/home health or related markets (2yr+) required. Robust healthcare understanding required. Self motivated with a desire to win. Comfortable with utilizing and interpreting data technology to drive business development. Excellent communication skills, both verbal and written, with the ability to educate diverse audiences effectively. This role requires a proactive individual who is committed to making a positive impact within the community through effective liaison activities.Job Type:
