We are looking for a professional who already has established connections with physicians, hospitals, skilled nursing facilities, assisted living facilities, case managers, discharge planners, and community referral sources. The ideal candidate understands hospice eligibility, can confidently educate referral partners about hospice services, and has demonstrated success in generating admissions through relationship-building and strategic account management. Responsibilities Develop and maintain relationships with: Physicians and physician groups Hospital discharge planners Skilled nursing facilities Assisted living facilities Memory care communities Case managers and social workers Community organizations and senior service providers Generate qualified hospice referrals and admissions. Educate referral sources on hospice services, eligibility criteria, and benefits. Conduct routine in-person visits with referral partners. Identify and develop new referral opportunities. Maintain and grow assigned territories. Attend community events, networking opportunities, and healthcare meetings. Track referral activity and maintain accurate documentation. Collaborate with clinical leadership to ensure seamless patient transitions and excellent customer service. Meet and exceed monthly admission and census growth goals. Qualifications Required Minimum 3 years of healthcare marketing experience. Minimum 2 years of hospice marketing experience. Established referral relationships within the healthcare community. Demonstrated success growing hospice admissions and census. Strong knowledge of hospice regulations and eligibility criteria. Excellent communication, presentation, and relationship-building skills. Reliable transportation and valid driver's license. Ability to work independently and manage a territory effectively. Preferred Existing relationships with hospitals, physicians, SNFs, ALFs, and community partners within the territory. Experience working with hospice EMR and CRM systems. Bachelor's degree in Healthcare Administration, Marketing, Business, or related field.
