  • 5:30 PM Rotating on-call support as needed Occasional emergency assessments after hours or weekends Travel Requirements This position does require travel throughout South Carolina for client assessments, supervisory visits, hospital discharges, and client care coordination.
Mileage and gas allowance are provided for approved work-related travel. Reliable transportation, valid driver's license, and the ability to travel consistently throughout the state are required. Position Overview Avodah Homecare is seeking a highly professional, organized, compassionate, and clinically experienced Senior Client Care RN/LPN Coordinator to join our leadership team in Columbia, South Carolina. This is a critical leadership role responsible for: Conducting client assessments Creating and updating care plans Supervising caregiver performance Coordinating services for elderly and disabled clients Supporting scheduling and operations Assisting families with high-level communication and problem-solving Ensuring quality assurance and compliance standards are maintained The ideal candidate is detail-oriented, dependable, calm under pressure, and passionate about delivering elite-level homecare services. This individual will serve as both a field clinician and an in-office coordinator, helping maintain excellence across Avodah Homecare operations.
Primary Responsibilities:
Client Assessments & Clinical Oversight Conduct initial in-home assessments for new clients Perform reassessments for existing clients Travel throughout South Carolina to complete assessments and supervisory visits Develop individualized care plans based on client needs Evaluate safety risks within the home environment Monitor changes in client condition and communicate updates appropriately Ensure proper documentation and compliance with company standards Conduct supervisory visits and caregiver competency evaluations Assist with care transitions from hospitals, rehab facilities, and hospice Participate in high-acuity and dementia care planning Coordination & Office Operations Act as an in-office coordinator supporting daily operations Assist with caregiver/client matching for successful placements Collaborate closely with scheduling and operations teams Support staffing coordination during call-outs and emergencies Maintain communication with families, hospitals, physicians, social workers, and referral sources Assist with resolving client concerns and service issues professionally Help ensure all client files, care notes, and documentation remain accurate and audit-ready Leadership & Team Support Mentor caregivers and provide clinical guidance Assist with caregiver orientation and training Support mandatory in-services and educational meetings Reinforce Avodah Homecare standards, professionalism, and customer service expectations Participate in quality assurance initiatives and performance improvement efforts Qualifications Required Active South Carolina RN or LPN License Minimum 2 years of nursing experience Minimum 1 year of homecare, hospice, assisted living, rehab, or long-term care experience Strong assessment and documentation skills Excellent communication and leadership abilities Professional appearance and demeanor Reliable transportation Valid driver's license Clean background check Ability to travel throughout South Carolina Ability to multitask in a fast-paced environment Preferred Experience with dementia and Alzheimer's care Experience with Medicaid, VA, private pay, or long-term care insurance clients Previous scheduling or coordinator experience Knowledge of caregiver supervision and homecare compliance Experience with high-acuity or 24-hour cases Skills We Are Looking For Strong critical thinking Excellent time management Leadership and accountability Compassion and emotional intelligence Problem-solving under pressure Professional phone etiquette Team-oriented mindset Attention to detail Ability to handle sensitive family situations with professionalism Why Join Avodah Homecare?
