$55,120

NATURE OF WORK
The Senior Life Solutions Primary Therapist provides comprehensive psychotherapy and social services to program participants. This role is responsible for conducting psychosocial assessments, developing and implementing treatment plans, leading individual, group, and family therapy sessions, and coordinating discharge planning. The therapist collaborates closely with interdisciplinary team members, ensures compliance with hospital, state, and federal standards, supervises social work interns, and contributes to ongoing quality improvement initiatives.
RESPONSIBILITIES
Conduct psychosocial assessments and develop the therapy components of individualized treatment plans Provide individual, group, and family therapy in accordance with patient needs and provider orders Maintain accurate, timely clinical documentation and treatment updates Coordinate discharge planning and ensure completion of all required documentation Collaborate with physicians, nurses, and interdisciplinary team members to support patient goals Uphold patient rights, confidentiality, dignity, and safety Represent the program at educational or community events as requested Assist with data collection, reporting, and quality improvement activities under the QAPI Plan Supervise and mentor social work interns
EDUCATION/QUALIFICATIONS
Required:
Master's degree in Social Work (MSW) from an accredited Graduate School of Social Work, or a related degree that supports licensure
Required:
LMSW, LCSW, LPC, or LMFT in the
State of Illinois Preferred:
2-3 years of clinical experience working with older adults
Preferred:
Prior experience in a hospital or nursing home setting; Familiarity with Joint Commission, Medicare, and state licensure standards.
EXPERIENCE
    Preferred:
    2-3 years of clinical experience with older adults
    • Hospital or nursing home experience strongly valued
    • Prior experience in treatment planning, discharge coordination, and interdisciplinary collaboration SKILLS Strong interviewing and assessment skills Ability to develop comprehensive psychosocial evaluations and treatment plans Proficiency in individual, group, and family therapy Excellent documentation, organization, and time‑management abilities Effective communication and collaboration with interdisciplinary teams Professionalism, integrity, and compassionate patient interaction Leadership skills for supervising interns and representing the program
