Senior Outreach & Engagement Case Manager 3.5 3.5 out of 5 stars Torrington, CT 06790 From $24 an hour - Full-time McCall Behavioral Health Network 4 reviews From $24 an hour - Full-time Senior Outreach & Engagement Case Manager The Senior Outreach and Engagement Program provides assessment and case management services to at-risk older adults (55 and older) by utilizing proactive approaches to identify, engage, and refer seniors for various individually tailored community treatment options. Services include education, support, counseling, referrals to senior service networks, and referrals for treatment. The program goals are to provide the services in a person-centered, strengths-based, culturally sensitive manner that reduces substance misuse, stabilizes behavioral health symptoms, and improves quality of life, while assisting the older adult with remaining integrated in the community in the least restrictive setting possible. Full-time/Non-Exempt Pay rate begins at $24.00 QUALIFICATIONS Bachelor's Degree in Human Services or related field preferred. Must have proven clinical, verbal, and written communication skills. Must be able to work independently and in the community. Ability to speak Spanish preferred.
Physical Requirements:
Prolonged periods of time sitting at a desk, using the phone, and working on a computer. Must be able to lift up to 20 pounds at times. Valid Driver's license required.BENEFITS 4

Assess clients by identifying and evaluating individual strengths, weaknesses, problems, and needs for the development of an individualized service plan. Develop service plans with clients by identifying and ranking problems needing resolution; establishing reasonable and achievable goals; determining a recovery process and resources to be used; and initiating discharge planning. Assist clients in gaining access to services, medical, social, educational, vocational, housing, and other services essential to meeting basic human needs and achieving optimal quality of life and community living. Advocate for clients in various systems to assure quality treatment and equity of care. Provide crisis prevention and intervention by identifying and helping clients to identify triggers for and/or symptoms of relapse, and early intervention to prevent further setbacks.Similar remote jobs
