Posting Text Job Summary/Basic Function:
The Case Manager coordinates and manages the Bear Care initiative. The Case Manager is a key staff position responsible for providing holistic wrap-around support services by assisting students with negotiating access to campus and community resources for behavioral, financial and social service needs. The Case Manager will provide assessment, case management, and follow-up support services and referrals to community services providers and college specific resources for students who are experiencing non-academic difficulties that may impede their persistence and academic success. Specifically, the Case Manager works with, supports, and connects students to resources; develops preventative education and response strategies; and tracks student outcomes. The Case Manager collaborates and coordinates with the Bear Care Advisory Committee and is a liaison between Bear Care and other campus offices (including Accommodative Services, Academic Advising Center, Center for Diversity, Counseling Center, Residence Life, Student Financial Services, Student Health Services, etc.) and community service providers. Job Requirements & Essential Functions of theJob:
Serve as a primary resource for students in distress, at risk, or in crisis as well as with students and their support networks (families, guardians, friends, etc.) following a crisis. This involves collecting, organizing, and evaluating facts associated with student cases; developing intervention plans; and communicating with the impacted student(s), their support networks, and administrators. Coordinate, collaborate and consult with key stakeholders, on and off campus, to develop a coordinated, multidisciplinary approach for responding to, advocating for, and supporting students, including supporting students returning from hospitalizations. Manage accurate record keeping through an electronic reporting database and in compliance with college, local, state, and federal guidelines on reporting; analyze and maintain data; prepare reports, statistics, and presentations documenting campus and national trends; maintain a comprehensive roster of students identified as being in distress or at risk. Support the Behavioral Evaluation Committee (BEC) in partnership with the Dean of Students, faculty, staff, and other students who are assisting students (i.e. friend groups, peer advisors and resident assistants). Coordinate evidence-based campus programming and training initiatives that address contemporary concerns in a college population such as substance use and abuse, healthy relationships, conflict management, stress reduction and other wellness topics. Provide consultation, guidance, and follow-up, for campus community members who are working with students of concern. Provide training, technical assistance, and consultation with members of the campus community on issues related to de-escalation, mediation, conflict resolution, bystander intervention, classroom disruption and other related issues. Work with community partners to compile & promote external support resources. Oversee the Bear Care Committee including coordinating meetings and activities, managing website, etc. Manage the campus food pantry and SNAP resources. Serve as the campus Homelessness Liaison Manage the campus Transportation Insecurity Task Force; develop and oversee campus student driver program to coordinate transportation for students to and from off-campus locations for appointments, services, etc. Connect students with health insurance needs with Health Navigators. Coordinate Student Affairs outreach to non-registered students and students with extensive midterm progress notes. Provide student support services to a diverse student population including non-traditional student learners, on-line learners, first generation college students, College in Prison program, etc. Research funding opportunities and prepare high‑quality grant proposals to obtain resources that support case management service priorities. Promote a campus and community awareness campaign designed to alert students, faculty, and staff to case management services through presentations, social media and webpage, and written materials at orientation, Student Government Association (SGA), and the shared governance structure. Oversight of Student Care Fund and processing of emergency funding requests. Provide services after hours as needed. Participate in overall activities and mission of Student Affairs including professional development and continuing education. Additional duties will include assisting with the College's support of other programs and initiatives.Required Qualifications:
Bachelor's degree from a four-year college or university is required. Minimum of 2 years relevant experience post bachelor's degree required. Excellent communication, interpersonal, and organizational skills are required. Ability to work under stress, to work independently and in a team environment. Must be able to assure confidentiality and secure documents and sensitive records. Must have excellent presentation skills. Proven ability to work with a diverse student population. Must be proficient with computer applications (Word, Excel and PowerPoint). Advanced degree from a four-year college or university in counseling, higher education or related field is preferred. Preference given to candidates with knowledge or experience related to: Creativity, flexibility and initiative in meeting the needs of a diverse campus community in triage or crisis management situations; Conflict resolution and mediation techniques; Engaging students through social media; Public speaking and presentation skills; Applicable local, state and federal laws including Title IX, Title VI, ADA, VAWA, Clery Act, and FERPA.
