Job Description

The Service Coordinator arranges authorized services to assigned program participants through the EMR system. Documentation, data entry, creating and updating spreadsheets, creating reports, tracking services, services delivery, communication with vendors, and participants is essential.
Duties Include:
Review the service request list to assess, contact participants and/or their assigned Support Coordinators to clarify their current need for services. Arrange for provider services as authorized, utilizing quality improvement initiatives. Communicate with providers, participants, caregivers, and staff as needed to arrange services or resolve issues. Coordinate and process all necessary participant correspondence pertaining to service coordination. Maintain knowledge of program changes and the latest processes through employee development activities and trainings. Create, update, and maintain reports and dashboards to track services, providers, other essential data, and identify trends. QUALIFICATIONS
High School Diploma or GED minimum requirement. Associate's Degree in Secretarial Science, Liberal Arts, or Health/Human Services preferred. Must have prior work experience in: office administration, health/human services or related field as follows:
HS/GED + 6
months -1 year experience or associate's degree + 1-2 years' experience. Must have excellent written and oral communication skills and strong organization skills. Great problem solving, critical thinking and time management skills. Must high level of accuracy, excellent data entry skills and type 40 wpm. Strong computer skills in Word, Excel and Outlook required.
