Service Coordinator

Service Coordinator - MCCA, Inc. MCCA, Inc., a premium substance use treatment and mental health organization, is seeking a Service Coordinator to manage referrals and coordinate discharge planning. This role supports client engagement, retention in treatment, and successful transition to the next level of care. Key Responsibilities Coordinate referrals and discharge planning in collaboration with clinical staff Meet weekly with clients to develop individualized aftercare plans Initiate and maintain discharge documentation in CareLogic Facilitate referrals to community, recovery, medical, and wellness resources Lead two client groups or workshops focused on transition, recovery supports, and wellness Encourage participation in recovery, wellness, and community activities Collaborate with counselors and external service providers to support client well‑being Provide peer‑based support, including accompaniment to appointments and activities Maintain accurate documentation and comply with HIPAA and confidentiality standards Assist with client and room searches; provide outreach and transportation as needed Minimum Qualifications Associate's or Bachelor's degree preferred in a social science-related field Strong communication and teamwork skills Proficiency in Microsoft Office and electronic medical records Valid Connecticut driver's license and access to a reliable vehicle
