Our Skilled Nursing Facility offers short-term rehabilitation and long-term care services, with a strong focus on individualized care and smooth transitions across care settings. We are seeking a detail-oriented, empathetic, and collaborative Discharge Planner to join our interdisciplinary team. This role plays a critical part in care coordination, discharge planning, and supporting the emotional and social well-being of our residents and their families.

Key Responsibilities:

Develop and implement individualized discharge plans in collaboration with residents, families, and the care team. Serve as a liaison between the facility, hospitals, home health, hospice, and other post-acute care providers. Coordinate transportation, home care, medical equipment, and follow-up appointments to ensure seamless transitions. Conduct assessments and maintain documentation in compliance with federal and state regulations. Advocate for residents' psychosocial needs and connect them with appropriate resources. Participate in care plan meetings and interdisciplinary team discussions.

Qualifications:

Prior experience in discharge planning or case management in a healthcare or SNF setting strongly preferred. Knowledge of Medicare, Medicaid, and insurance authorizations. Strong communication, organizational, and problem-solving skills. Ability to work collaboratively in a fast-paced environment.
