Position Summary The Community Strategist will learn and understand available community resources and programs; utilize data and local knowledge to build grassroots, strategic, and data informed community partnerships; and identify opportunities to develop and pilot new SDoH ( Social Determinants of Health ) initiatives. Job Duties include (but are not limited to): Implement and scale the overall strategy to address Health Equity & SDoH for members and communities in assigned region. Operationalize SDoH product in assigned region. Learn and understand available community resources and programs. Utilize data and local knowledge to build grassroots, strategic, and data informed community partnerships. Identify opportunities to develop and pilot new Health Equity & SDoH initiatives. Build partnerships to incorporate Health Equity solutions into SDoH, population health, care management, quality, regulatory, and community outreach programs. Establish outcomes-based contracts with community-based organizations. Organize community activation meetings to successfully execute key Health Equity & SDoH initiatives with community partners. Create and support Business Development initiatives to align with Health Equity & SDoH strategy. Cultivate and foster relationships with internal and external stakeholders to drive collective member impact. Monitor and track Health Equity & SDoH data and deliverables. Serve as a public speaker at coalitions, conferences etc. to positively position the organization. Required Qualifications 3 + years of experience with health equity, population health, care coordination, and/or community-based programs. Must reside in Illinois, specifically in one of the following counties: Pike, Scott, Morgan, Sangamon, Christian, Macon, Shelby, Cumberland, Clark, Calhoun, Greene, Jersey, Macoupin, Montgomery, Fayette, Effingham, Jasper, Crawford, Madison, Bond, Saint Clair, Monroe, Clinton, Washington, Marion, Clay, Richland, Lawrence, Washington, Jefferson, Wayne, Edwards, Wabash, Randolph, Perry, Franklin, Hamilton, White, Jackson, Williamson, Saline, Gallatin, Union, Johnson, Pope, Hardin, Alexander, Pulaski, or Massac. Ability to work Monday-Friday from 8am-5pm Central Standard Time, with the ability to be flexible beyond core hours as needed. Must possess reliable transportation and be willing and able to travel up to 50% of the time within southern Illinois. Mileage is reimbursed per our company expense reimbursement policy. Preferred Qualifications Prior experience with managed care. Excellent interpersonal and communication skills. Demonstrates good oral and written presentation skills. Strong organizational skills, including the ability to prioritize, multi-task, and manage workload to meet specific timeframes and deadlines. Education Bachelor's Degree in health administration, social services, public health, business administration, psychology, or closely related field. Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $46,988.00 - $112,200.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
