Job Description

Social Services Director Indianapolis, IN Full-time Full-time Make a Meaningful Impact Every Day At Outfield Healthcare Partners , we believe exceptional care starts with compassionate people. We are seeking a Social Services Director to support the emotional, social, and psychological well-being of our residents through leadership, advocacy, and care planning. What You'll Do Complete psychosocial assessments and create individualized care plans Provide emotional and mental health support to residents and families Lead the discharge planning process and coordinate community referrals Uphold and advocate for residents' rights and dignity Collaborate with nursing and interdisciplinary care teams Maintain timely, accurate documentation that meets all state and federal requirements Participate in regulatory survey preparation and execution Support or supervise social services staff, if applicable
Qualifications Required :
Bachelor's Degree in Social Work or related Human Services field (Psychology, Sociology, Rehab Counseling, etc.)
Required :
Certified, licensed, or registered to practice in
Indiana Required :
1 year of supervised social work in a healthcare setting (geriatrics preferred)
Preferred :
