A DAY IN THE LIFE OF A SOCIAL SERVICES DESIGNEE
As a Social Services Designee, you play a vital role in supporting residents' emotional, social, and behavioral well-being. Each day, you help ensure residents feel heard, supported, and connected.
Your daily responsibilities include:
Conducting psychosocial assessments to evaluate resident emotional and social needs Providing emotional support, active listening, and crisis intervention when needed Advocating for resident rights, preferences, and person-centered care Developing and updating individualized service plans focused on mental health and social needs Coordinating with families, guardians, mental health providers, and outside agencies Monitoring changes in resident mood, behavior, and functioning Supporting behavioral interventions and care planning initiatives Assisting with admissions, transitions, and discharge planning to ensure continuity of care Maintaining accurate documentation in compliance with facility policies and state regulations Your ability to connect with residents, collaborate with care teams, and advocate for individualized support is what makes this role essential to our mission. Qualifications Education & Experience Bachelor's degree in Social Work, Psychology, Human Services, or related field preferred Experience working with individuals with mental illness, behavioral health needs, or in assisted living/long-term care settings strongly preferred Skills & Competencies Strong understanding of mental health diagnoses, symptoms, and interventions Ability to de-escalate crisis situations and manage challenging behaviors Excellent communication, organization, and interpersonal skills Ability to work collaboratively with multidisciplinary teams Working Conditions Frequent interaction with residents experiencing mental health symptoms such as anxiety, depression, psychosis, or behavioral challenges May require response to urgent or crisis situations Combination of office work and time spent in resident living areas
