Community‑Based & Facility‑Based Settings Position Summary The Employment Coordinator is responsible for planning, coordinating, and overseeing employment services for members receiving Pathways to Work (PTW), Employment Support Aide (ESA), Group Supported Employment (GSE), and Individual Supported Employment (ISE) under the Arizona DES/DDD Qualified Vendor system. This role ensures that all employment services are delivered in accordance with DDD service specifications, ISP goals, HCBS requirements, and agency policies. The Employment Coordinator provides direct support, job development, employer engagement, documentation oversight, and service coordination to help members obtain and maintain competitive or group‑based employment. Essential Duties & Responsibilities 1. Service Coordination & Member Support Develop employment curriculum as per DDD requirements. Conduct intake assessments to identify strengths, interests, barriers, and employment goals. Develop individualized employment plans aligned with the member's ISP and DDD service definitions. Coordinate PTW, ESA, GSE, and ISE services to ensure continuity and progression toward competitive employment. Provide on‑site and community‑based support, including job coaching, skill‑building, and workplace acclimation. Support members in developing soft skills, communication, self‑advocacy, and workplace behavior. Monitor member progress and adjust strategies as needed. 2. Job Development & Employer Engagement Build and maintain relationships with local employers, HR departments, and community partners. Conduct job searches, employer outreach, and labor‑market matching based on member strengths. Advocate for reasonable accommodations and inclusive hiring practices. Coordinate interviews, job trials, and onboarding support. Promote natural supports within the workplace to increase independence. 3. Program Oversight & Compliance Ensure all services meet DDD Qualified Vendor standards and HCBS requirements. Maintain accurate, timely documentation including daily notes, service delivery logs, and progress summaries. Ensure billing units match service delivery and comply with DDD rate book guidelines. Participate in ISP meetings, team meetings, and interdisciplinary planning. Maintain confidentiality and adhere to HIPAA, DES/DDD, and agency policies. 4. Training & Support for Employment Staff Provide guidance and support to Job Coaches, ESA staff, and GSE team members. Train staff on DDD service expectations, documentation standards, and workplace safety. Assist with scheduling, coverage, and coordination of employment‑related activities. 5. Transportation & Community Integration Provide or coordinate transportation to interviews, job sites, community activities, and employment‑related appointments. Support members in navigating public transportation when appropriate. Promote community inclusion and independence in all employment settings. Required Qualifications High school diploma or GED required; Associate's or Bachelor's degree preferred (Human Services, Psychology, Education, or related field). Minimum 3-4 year of experience providing DDD employment services (PTW, ESA, GSE, or ISE). Knowledge of DES/DDD service specifications, ISP process, and HCBS rules. Valid driver's license, reliable transportation, and acceptable driving record. Level One Fingerprint Clearance Card (required). CPR/First Aid certification (or ability to obtain). Article 9 certification (or ability to obtain). Strong communication, documentation, and organizational skills. Preferred Qualifications Experience with job coaching, vocational rehabilitation, or supported employment. Familiarity with behavioral support strategies and trauma‑informed care. Experience working with individuals with developmental disabilities in community settings. Bilingual (English/Spanish) preferred but not required.
